The form must be signed, dated, and returned by mail or in person to be valid. Click here to download a copy of this form. Upon completion, return the signed and dated form to Silver Cross Hospital, Medical Records Department, 1200 Maple Rd., Joliet, IL 60432. Note: You may leave the last line of the form and the number of pages released blank; Silver Cross Hospital will complete this information when your records are released. For a copy of your birth certificate you need to contact the Will County Clerk's Office at (815) 740-4615.
