For new and existing patients, please bring these items with you on the day of your visit:

- Health Insurance Card/Information
- A friend or family member to take notes/help with questions
- A list of your questions /concerns
- A list of all medications you are taking
- A copy of your X-RAYS / CT scans (actual films and reports)
- Copies of records from doctors related to your sinuses/nose/allergies/lungs.

If You Require Hospitalization
We have a team of dedicated hospitalists: doctors and advanced practice providers who specialize in the care of hospitalized patients. They are board-certified, highly trained and maintain the highest standards of care. So you can rest assured you will receive the very best care during your stay at Silver Cross Hospital.
Our hospitalists communicate regularly with your regular doctor or advanced practice provider, giving important updates on your condition, plan of care and discharge. They are excellent resources for you and your loved ones too and can answer any questions you may have.
