The Diabetes Center of Excellence at Silver Cross Hospital began in 1996 with a commitment to promote health and well-being and act as a resource center for individuals with diabetes and their family members.
To this day, a team of specially trained nurses and dietitians work closely with patients and their physicians to develop a personalized program, teaching participants essential information and skills for managing diabetes.
What We Provide
The Silver Cross Diabetes Center educational program is one of the first in Northern Illinois recognized by the American Diabetes Association. This recognition formally identifies the Diabetes Center as one of excellence in performance due to its quality educational services to the patients in the community it serves. The ongoing program of treatment and support includes everything from medical management to meal planning and lifestyle management.
The Diabetes Center offers the following services individually or through class sessions:
• Medication Management/Insulin Instruction
• Insulin Pump Therapy
• Sensor Training, CGMS (Continuous Glucose Monitoring)
• Self Blood Glucose Monitoring
• Meal Planning/Weight Management
• Exercise Planning
• Management of Gestational Diabetes
