COVID-19 Vaccine Clinic
If you’ve been waiting to get your COVID-19 vaccine, we’ve got good news to share!
The Will County Health Department has allocated COVID-19 vaccines so Silver Cross Hospital can vaccinate individuals ages 12 years and older. Availability is on a first-come, first-served basis. Please note: Any vaccine recipient between the ages of 12 and 17 MUST have a parent or legal guardian present at the time of vaccination.
* Per the Centers for Disease Control & Prevention, if you've had any other vaccines within the last 14 days, you should wait to get the COVID vaccine.

If you've already received your confirmation email, please click here to download, print and complete your consent form, which you will bring to your appointment.
Please note, your insurance may be billed for the cost of administering the vaccine, but you will not be required to make any payment (for example co-pay, co-insurance, deductible, etc.) Please bring a government-issued identification card and insurance card (if applicable) to your appointment.
